In October 2020, he made a pivotal move to the Manchester United Academy for a £420,000 fee. Garnacho signed his inaugural professional contract with the club in July 2021. His notable achievements include a standout solo goal in the FA Youth Cup against Everton, earning a nomination for United's Goal of the Month award in February 2022. After serving as an unused substitute in various Premier League matches, Garnacho made his first-team debut for United in April 2022, securing the Jimmy Murphy Young Player of the Year award in May and contributing two goals to the FA Youth Cup final victory against Nottingham Forest on May 11, marking United's first win in the competition since 2011.

In the 2022–23 Premier League season, he made significant strides for Manchester United. Transitioning from shirt number 75 to 49, he showcased his prowess by scoring a late winner against Barrow in the 2022–23 EFL Trophy. Garnacho earned his first senior start on October 27 in a 3–0 UEFA Europa League victory over Sheriff Tiraspol, drawing praise from manager Erik ten Hag for his improved attitude. Notably, he scored his first senior goal against Real Sociedad in the Europa League with an assist from his idol Cristiano Ronaldo and later secured a Premier League injury-time winner against Fulham on November 13.

In the Manchester derby on January 14, 2023, Garnacho played a pivotal role, setting up Marcus Rashford's 82nd-minute winner. As the 2023–24 season commenced, his squad number changed to 17, and he made headlines with a controversially disallowed late goal against Arsenal on September 3.

However, Garnacho quickly rebounded, scoring a spectacular bicycle kick on November 26 to open the scoring in a 3–0 win over Everton. Manchester United praised Alejandro Garnacho for his remarkable overhead wonder goal against Everton, which they deemed "better than Rooney's," while the winger himself drew comparisons to a goal scored by Cristiano Ronaldo. Garnacho shared a side-by-side image of his acrobatic opener in Sunday's 3-0 victory against Everton with a well-known similar goal from his idol Ronaldo in 2018.

Alejandro is eligible to play for both Spain and Argentina. He started his international career in 2021 with three appearances for the Spain under-18 team. His international journey continued as he earned a call-up to the senior Argentina team in March 2022, making it to the final 33-man squad but not featuring in the World Cup qualifiers. Garnacho's impressive performance for the Argentina under-20 side in the 2022 Maurice Revello Tournament, where he scored four goals and received accolades such as the Revelation Player and Goal of the Tournament awards, solidified his standing. Despite a setback with an ankle injury in March 2023, he made his senior team debut on June 15, 2023, during a friendly against Australia, entering the field as a substitute for Nicolás González in the second half at Workers' Stadium.
